Summary DWC is seeking a full-time Day Center Case Manager to provide comprehensive, trauma-informed care to unaccompanied women and women with children experiencing homelessness. The Day Center Case Manager delivers services with a trauma informed approach and is a patient supporter who actively listens and maintains appropriate boundaries. This position connects clients with services and resources, serves as an advocate and collaborates across departments to meet the immediate and long-term needs of each woman. The Day Center Case Manager is a member of the Clinical Department and reports directly to the Clinical Program Manager.
Essential Functions
Provide trauma-informed case management services to clients
Create individual service plans with clients that address their needs and goals
Identify and provide linkage to needed resources (basic needs, health & mental health, benefits, housing, legal support, etc.)
Serve as an advocate and liaison for clients in accessing local resources
Provide effective crisis intervention in consultation with program staff and supervisor
Develop and maintain relationships with clients, staff and local service providers
Administer the VI-SPDAT survey as needed and assist clients in identifying permanent housing opportunities
In accord with DWC’s documentation policy and per contract requirements, maintain accurate and timely records of all services provided to clients in the Clarity/HMIS database, including case management notes
Enroll women who have experienced trauma related to sexual assault, domestic violence, and human trafficking crimes in DWC’s Trauma Recovery Center (TRC) program.
Attend multi-disciplinary team meetings and regular one-on-one supervision as scheduled
Track outcomes and monitor progress towards program goals
Serve as an ambassador of DWC and its mission and values by participating in community meetings/events
Provide regular, timely written reports to supervisor as assigned

Work EnvironmentThis job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision and ability to adjust focus. This would require the ability to lift files, open filing cabinets and bend or stand on a stool as necessary.
Position Type and Expected Hours of Work
This is a full-time, non-exempt position. Days and hours of work are Monday- Friday 8:00AM- 4:30PM.
Travel Regular travel throughout Los Angeles County is required in this position.
Remote
Option for hybrid work schedule but with expectation to work onsite full time as scheduled.
Required Education And Experience
Bachelor’s degree in social work/related field or relevant experience preferred
Minimum two years of experience working as a Case Manager/similar position and knowledge of community resources preferred
Experience working with individuals experiencing homelessness preferred
Excellent active listening and crisis intervention skills with the ability to exercise and model good judgment
Strong interpersonal, written and oral communication skills; proven ability to communicate with diverse audiences
Ability to manage multiple tasks including proficiency in balancing direct service responsibilities with effective record keeping
Detail-oriented, highly organized, able to manage multiple tasks and priorities, set and meet goals and deadlines
Strong knowledge of the complexity of poverty and women’s homelessness, mental illness, trauma, substance abuse, and health issues
Able to solve problems by being resourceful and using creativity
Ability to work independently and as part of a team
Oral and written fluency in English required; Spanish a plus
Proficiency in Microsoft Office Suite (Word, Excel, Outlook) required
Must be a CA resident and live within 80 miles from the office and must be available for in-person meetings at the DWC offices.
Must provide documentation of a negative TB test result within the last twelve months, on first day of hire.
You will be required to have received at least the first dose of COVID-19 vaccination to start employment with DWC, and second dose must be received within 30 days from the first day of hire.
